Role of Rotational Atherectomy in Percutaneous Coronary Interventions in Elderly

چکیده

RA is a debulking device that physically removes plaque and reduces plaque rigidity basically altering vessel compliance, similar to that achieved by a dental drill. It works on the principle of “differential cutting”, ablating only the calcified and fibrotic “non-compliant” tissue, while retaining the elastic arterial wall. The hard and calcified plaque is not able to deflect the rota-burr and gets ablated producing microfissures at the contact zone. The “ablated” plaque is broken into particles that are smaller than 5uM in diameter that pass through the vasculature and absorbed by the Reticulo-Endothelial System [4].

A case of an entrapped rotational atherectomy burr.

Rotational atherectomy is a widely used modality in percutaneous coronary revascularization, especially for heavily calcified coronary arteries [1–5]. Due to the unique mechanism of rotational atherectomy, there have been reports of unusual complications, including fracture of coronary wire, guidewire bias, coronary spasm leading to myocardial infarction, and fracture of the drive shaft causing...

Current status of rotational atherectomy.

Despite the increasing use of percutaneous transluminal coronary angioplasty and intracoronary stent placement for the treatment of obstructive coronary artery disease, a large subset of coronary lesions cannot be adequately treated with balloon angioplasty and/or intracoronary stenting alone. Such lesions are often heavily calcified or fibrotic and undilatable with the present balloon technolo...

Rotational atherectomy via transradial access in the percutaneous treatment of chronic coronary total occlusion with rare complication of coronary guidewire transection

Introduction C total coronary occlusion remains one of the limitations of percutaneous transluminal coronary angioplasty, and few therapeutic devices are specifically designed to address this problem. Despite advances in device technology, the management of resistant, calcific lesions remains one of the greatest challenges in successful chronic total coronary occlusion intervention. Established...

Percutaneous retrieval of a detached rotational atherectomy burr

Rotablation (rotational atherectomy) is an acknowledged method of percutaneous treatment of highly calcified coronary artery lesions that cannot be treated with traditional angioplasty. The complexity of the technique and usage of very specific equipment can contribute to the development of uncommon complications. We present a case of percutaneous retrieval of a damaged rotational atherectomy b...
